Selective Noise Reduction

First, thanks Vlad for your previous answers--very helpful. Second: it's often said that noise reduction can be more harmful than helpful (not NI, of course ). My question is, how often do you experts just noise reduce locally, let's say to non-detailed areas like sky, large shadow areas etc? Is this program so good that reducing globally is usually just fine? And lastly: when do you use this in the workflow: early, or at the end of the workflow...after resizing for printing and just before output sharpening?
